As part of the West Cook Coalition (WCC), Community Support Services receives funding through the Illinois Department of Healthcare and Family Services (HFS) as part of the Healthcare Transformation Collaborative. WCC is focused on innovative visions and equity focused solutions in support of its goal to reimagine healthcare in Illinois.
Through this coalition, starting in September 2023, CSS has a funded Community Healthcare Worker to support individuals with an intellectual/developmental disability (I/DD) and their family. The Community Healthcare Worker is focused on advocacy, referrals, and supportive case management to decrease the barriers and inequities individuals with I/DD face. The Community Healthcare Worker will advocate, make referrals, and support individuals with an I/DD and their family in all areas of healthcare including medical, dental, and psychiatric care. Services may be provided one time or as needed on an ongoing basis. CSS offers a continuum of care leading to improved health outcomes for individuals with an I/DD.
