CSS was awarded a grant by the Illinois Department of Healthcare and Family Services (IDHS). IDHS chose six healthcare collaboratives for the State Fiscal Year 2023 cycle of the Healthcare Transformation Collaboratives. CSS is the only I/DD provider included in the West Cook Coalition, a group of 25community partners led by Loyola University Medical Center. This collaborative will target the health inequities and poor health outcomes that exist in itsservice area, which includes 18 zip codes in western Cook County, by putting inplace a care management model that aims to reach high-risk and rising risk Medicaid customers and uninsured adults.
